 - doesn't distinguish between per mount and per super block options

This patch introduces /proc/<pid>/mountinfo which attempts to address
all these deficiencies.

Code shared between /proc/<pid>/mounts and /proc/<pid>/mountinfo is
extracted into separate functions.
